The number of skilled nursing hours approved is individualized — determined by the clinical documentation reviewed by Alliant Health Solutions. Many vent-dependent children qualify for high-acuity authorizations, including authorization for around-the-clock skilled nursing.

Hospital Discharge
Transitioning Home From the Hospital
How Core Health Group Corp. coordinates a safe discharge for vent-dependent children.
Many ventilator-dependent children transition to GAPP after an extended hospital stay. Core Health Group Corp. works with hospital discharge planners, pulmonologists, and intensivists to ensure that the home environment, equipment, and nursing care plan are in place before discharge.
Pre-discharge care planning
Core Health Group Corp. coordinates with the hospital care team — including pulmonologists and intensivists — to develop the home nursing plan before discharge.
Home assessment and equipment coordination
We work with the DME vendor to confirm that ventilator equipment, backup power, and supplies are in place before the child arrives home.
Authorization submission and approval
The prior authorization packet is submitted to Alliant Health Solutions with complete clinical documentation to ensure authorized nursing hours are in place.
Staff assignment and training
Nurses are assigned and receive orientation specific to the child’s equipment, protocols, and emergency procedures before the first shift.
Care begins at home
Skilled nursing begins on the day of discharge. Core Health Group Corp. remains in close contact with the family during the initial transition period.

No minimum age. GAPP serves children from birth through age 20 years, 11 months. We routinely serve infants and toddlers with ventilator dependence.
We assign RNs or LPNs with demonstrated experience in home mechanical ventilation. All clinical staff receive orientation specific to each patient’s equipment, protocols, and emergency procedures.
