needs to feel confident.

Guides, FAQs, referral tools, and forms — all in one place to help families, caregivers, and healthcare partners navigate the home care process with clarity.

Getting Started

How home care works.

A simple, three-step process from first contact to active care.

01
How Home Care Works

A licensed caregiver or nurse visits your home on a scheduled basis to provide personal care, skilled nursing, or companionship services. Your care team is matched to your family's specific needs.

02
What to Expect During Intake

Your intake begins with a free consultation where we assess your loved one's medical needs, schedule, and home environment. We use this to build a personalized care plan before services start.

03
Care Plan Creation

Your care coordinator designs a written plan covering services, schedule, caregiver qualifications, and goals. The plan is reviewed with your family and updated as needs change.

Frequently Asked Questions

Common questions, honest answers.

Do you accept Medicaid?

Yes. We are an approved Georgia Medicaid provider and a certified GAPP (Georgia Pediatric Program) provider for medically fragile children and youth.

What areas do you serve?

We serve families across the Metro Atlanta region and surrounding Georgia counties, and we are actively expanding. Contact us if you're unsure whether we reach your area — we may still be able to help.

How quickly can care begin?

Most families can begin receiving care within 24 to 48 hours after intake is completed and a care plan is in place. Urgent cases are prioritized whenever possible.

Are your caregivers certified and background-checked?

Yes. All Core Health Group caregivers and nurses are background-checked, reference-verified, and hold appropriate certifications for their role. We maintain DCH licensure and full insurance coverage.

Do you offer pediatric home care services?

Yes. We specialize in pediatric home care through the GAPP Medicaid waiver program. Our caregivers and nurses are experienced in caring for medically fragile children and youth in the home setting.

Is care available 24/7?

We offer flexible scheduling including daytime, overnight, and live-in care options. Our care coordination team is available around the clock for urgent needs and support.

Can I choose my caregiver?

We work to match each client with a caregiver who fits their personality, schedule, and care needs. If a match isn't the right fit, we will work with you to find a better one.

What happens if my caregiver is sick?

We have a coverage team in place for situations when a scheduled caregiver is unavailable. Your care coordinator will notify you and arrange a qualified substitute as quickly as possible.
