Acceptance of Terms

By accessing or using the Core Health Group website at thecorehealthco.com (the "Site"), submitting any form, or engaging our services, you agree to be bound by these Terms of Service ("Terms") and our Privacy Policy, which is incorporated herein by reference.

These Terms constitute a legally binding agreement between you and Core Health Group, LLC ("Core Health Group," "we," "us," or "our"), a Georgia limited liability company with its principal place of business at 6065 Roswell Rd., #405, Atlanta, GA 30328.

If you do not agree to these Terms, you must not use our Site or services. We reserve the right to update these Terms at any time. Continued use of the Site after any changes constitutes acceptance of the revised Terms.

Description of Services

Core Health Group is a licensed home care agency operating under the Georgia Department of Community Health (DCH). We provide the following services to eligible individuals in Georgia:

Personal Care — Assistance with activities of daily living including bathing, grooming, dressing, mobility, and meal preparation.

Skilled Nursing — Licensed RN and LPN services including medication management, wound care, health assessments, and clinical oversight.

Companionship — Social engagement, light household tasks, and supportive presence for clients who benefit from human connection.

Disability Support — Pediatric, adult, and adaptive care services for individuals with physical or developmental disabilities.

Service availability is subject to geographic coverage, client eligibility, Medicaid or GAPP authorization where applicable, and staff availability. We reserve the right to modify, suspend, or discontinue any service at our discretion.

Eligibility & Service Agreement

To receive home care services from Core Health Group, clients must:

- Reside within our current Georgia service area - Complete a patient intake assessment - Meet clinical and eligibility criteria for the requested service - Execute a formal Service Agreement with Core Health Group prior to the commencement of services

Submission of an inquiry form, consultation request, or patient intake form on this Site does not create a service relationship or guarantee service delivery. A service relationship is established only upon execution of a signed Service Agreement.

For Medicaid and GAPP-funded services, eligibility is determined by the Georgia Department of Community Health and applicable program administrators. Core Health Group is not responsible for third-party eligibility determinations.

Permitted Use of This Website

You may use this Site for lawful purposes only. By using this Site, you agree that you will not:

- Use the Site in any way that violates applicable federal, state, or local law or regulation - Transmit any unsolicited or unauthorized advertising or promotional material - Attempt to gain unauthorized access to any portion of the Site or its related systems - Use any automated means (bots, scrapers, crawlers) to access, monitor, or copy content from the Site without our prior written consent - Impersonate any person or entity, or falsely represent your affiliation with any person or entity - Upload or transmit any material that is harmful, offensive, defamatory, or infringes on the rights of others - Interfere with or disrupt the integrity or performance of the Site

We reserve the right to terminate your access to the Site at any time, without notice, for conduct that we believe violates these Terms or is harmful to other users, Core Health Group, or third parties.

Intellectual Property

All content on this Site — including but not limited to text, graphics, logos, images, photographs, button icons, audio clips, data compilations, and software — is the property of Core Health Group or its content suppliers and is protected by United States and international copyright, trademark, and intellectual property laws.

The Core Health Group name, logo, and all related names, logos, product and service names, designs, and slogans are trademarks of Core Health Group, LLC. You may not use such marks without prior written permission from Core Health Group.

You are granted a limited, non-exclusive, non-transferable license to access and use this Site for personal, non-commercial purposes only. This license does not include: any resale or commercial use of the Site or its content; any collection and use of product listings or descriptions; any derivative use of the Site or its content; or any use of data mining, robots, or similar data gathering tools.

Any unauthorized use of the Site's content terminates this license.

Forms & Submissions

When you submit information through our website forms — including consultation requests, patient intake forms, callback requests, or career applications — you represent and warrant that:

- All information provided is accurate, current, and complete - You have the legal right to submit the information provided - You are at least 18 years of age, or are a parent or legal guardian submitting on behalf of a minor

By submitting a form, you consent to Core Health Group contacting you by phone, email, or text message at the contact information you provide, for the purpose of responding to your inquiry or processing your request. Standard message and data rates may apply.

We process form submissions through our CRM platform (GoHighLevel/LeadConnector) and workflow automation tools. Please see our Privacy Policy for details on how submitted information is handled.

Payment Terms

Payment terms for home care services are set forth in each client's individual Service Agreement. The following general terms apply:

Medicaid & GAPP Services — For clients with Medicaid or GAPP authorization, payment is processed through the applicable state program. Clients are responsible for any co-payments, spend-downs, or non-covered services as determined by their program.

Private Pay Services — Clients paying privately are responsible for service fees as set forth in the Service Agreement. Invoices are issued weekly or bi-weekly as agreed. Payment is due within 15 days of invoice date.

Non-Payment — Core Health Group reserves the right to suspend or discontinue services for non-payment, subject to applicable law and the terms of the Service Agreement.

Rate Changes — We may adjust service rates with 30 days' written notice to active clients. Continued receipt of services after the notice period constitutes acceptance of the new rates.

In no event shall Core Health Group's total liability to you for all claims arising out of or relating to the use of this Site exceed the greater of (a) the amount you paid to Core Health Group in the twelve months preceding the claim, or (b) one hundred dollars ($100).

Some jurisdictions do not allow the exclusion or limitation of certain types of liability. In such jurisdictions, our liability is limited to the maximum extent permitted by law.

Indemnification

You agree to defend, indemnify, and hold harmless Core Health Group and its officers, directors, employees, agents, and affiliates from and against any and all claims, damages, obligations, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising from:

- Your use of and access to this Site - Your violation of any of these Terms - Your violation of any third-party right, including any intellectual property, privacy, or proprietary right - Any claim that your submitted content or information caused damage to a third party

This defense and indemnification obligation will survive these Terms and your use of the Site.

Governing Law & Disputes

These Terms are governed by and construed in accordance with the laws of the State of Georgia, without regard to its conflict of law provisions.

Any dispute, claim, or controversy arising out of or relating to these Terms or the use of this Site shall be subject to the exclusive jurisdiction of the state and federal courts located in Fulton County, Georgia. You consent to personal jurisdiction in those courts.

Informal Resolution — Before initiating any legal proceeding, you agree to first contact Core Health Group at Info@TheCoreHealthCo.com and attempt to resolve the dispute informally. We will make good-faith efforts to resolve the matter within 30 days.

Waiver of Class Action — To the extent permitted by law, you waive any right to bring or participate in a class action lawsuit or class-wide arbitration against Core Health Group.

Termination

Core Health Group reserves the right, in its sole discretion, to terminate or suspend your access to all or part of this Site at any time, with or without cause, and with or without notice.

Upon termination, your right to use the Site ceases immediately. Provisions of these Terms that by their nature should survive termination — including intellectual property, disclaimers, indemnification, and limitation of liability — shall survive.

If you wish to terminate your relationship with Core Health Group as a client, please refer to the termination provisions in your Service Agreement. Website account termination and service termination are separate matters.

If any provision of these Terms is held to be invalid, illegal, or unenforceable, the remaining provisions shall continue in full force and effect.

Our failure to enforce any right or provision of these Terms shall not be considered a waiver of those rights.
