How to Refer a Family to Core Health Group Corp.
Follow these four steps to refer a GAPP-eligible child to our care team:
Confirm GAPP Eligibility
Verify the child is under 21, has active Georgia Medicaid (or is pursuing Katie Beckett / TEFRA enrollment), and has a diagnosis requiring skilled nursing or personal care. If eligibility is uncertain, contact us — we can assist with eligibility verification.
Provide Family Contact Information
Share the family's name, phone number, and the child's Georgia Medicaid ID with our intake team. We will contact the family directly to begin the intake process.
Share Clinical Information
A referral summary, recent physician notes, or hospital records (with authorization) help us begin the authorization packet preparation. The more clinical detail we have early, the faster the process moves.
Coordinate Authorization
We coordinate directly with the attending physician and any relevant specialists to complete the prior authorization submission to Alliant Health Solutions. We will keep you updated on status.

Yes. Families have the right to choose their GAPP provider at any time. The existing authorization transfers — no reauthorization is needed. We coordinate the transition directly with the current provider.
What documentation is needed for a referral?
At minimum: family contact info, Georgia Medicaid ID, primary diagnosis, and attending physician contact. Clinical notes or a recent physician summary expedite the process significantly.
